David A. Perez

David A. Perez, 52 Warsaw formally of Ligonier Indiana and Salina, Kansas passed away Sunday September 6, 2015 at 6:20 p.m. at Parkview Regional Medical Center, Fort Wayne, IN.He was born on September 30, 1962 to Andrew and Dorothy (Rice) Perez in Columbus Georgia. He spent most of his life in Salina, Kansas. He was an avid fan of the Kansas City Chiefs. David is survived by his 2 sons, Brian (and spouse Anni) Perez and Nathan Perez both of Warsaw; 2 sisters, Carmen Siemsen and Cathy Leonard both of Salina Kansas; a brother, Bill Perez of Salina, Kansas; a grandson Allen Perez of Warsaw. He is preceded in death by his parents and a son. There will be private calling and service at a later date.
